Final Funding Report
2025-2026
As a part of our fiscal responsibility, as well as our obligation to the numerous donors, who helped Hayward Education Foundation (HEF) fund your project, we are asking you to do your part as a funded recipient and complete an HEF final report at the end of your project as indicated below:
1. Fill out the funding report below.
2. Upload scanned copies of all receipts.
3. Be sure to have complete information for the following questions:

How did you achieve the expected results?

What were some of the important outcomes?

In what ways did student skills and knowledge increase?
4. Include photographs capturing your project; short videos if you have them.
5. Mail any unspent funds to HEF, PO Box 56444, Hayward, CA 94542 by May 15, 2026.
A complete report will greatly enhance HEF’s ability to explain to our donors the value of our work and the need for future funding.
